What is the worst agony a gout sufferer experiences? Is it the burning pain? Is it the swelling? The restraint caused by the pain of moving? Surely, it is the combination of all of them. Chronic gout, as its name suggests, is believed to be gout that will NEVER go away, in a sense that a person will get gout flare-ups all through-out his lifetime (believe me when I say that not all beliefs are true).

To those who is not aware of gout, it is a very cruel disease characterized by extreme burning pain and usually swelling in the affected joint. It can be caused by several factors revolving around uric acid, such as the Lesch-Nyhann Syndrome. It can also be caused by too much purine in the diet, because uric acid comes from purine.

So how does gout happens?

Ironically, this giant of a pain originates from something very small. The cause is even literally microscopic, namely URIC ACID CRYSTALS.

Normally, the liver, along with its friends in the excretory system, can dispose uric acid without difficulty. However, if the task at hand is heavy (such as when other ACIDS are to be flushed out), it is another story. Uric acid has to be temporarily stored while they can not be flushed out by the liver, and so that the body will not suffer from high acidity.

At first, the uric acid is stored at the interstitial fluids, THE HABITAT OF YOUR CELL (the thought of an acidic environment or your cells is fear-inducing. Imagine yourself swimming in a vat of acids).
Once the interstitial space is full, uric acid will be forced deeper into places that are not supposed to contain them, such as the synovial fluids in your joints. Eventually, uric acid will crystallize, your neutrophils attacking them as you cringe in pain. Ouch. Gout attack.

What if gout becomes EVERLASTING? In a sense that you will suffer from chains of gout attack for the rest of your life. Unfortunately, it is possible. Worse, the uric acid crystals begin showing itself piercing the skin!

Tophus (or tophi when plural) is the term used for the uric acid crystal, usually when it begins to protrude. It can appear not only in the joints but also in the earlobes, anywhere in the skin and in very rare cases, in the eyelids, spine, inside the ear and even in the male private part! Oucch.  Surely, it is painful and very obstructing.

Why Does Chronic Gout Happens?

A common cause of gout is the Lesch-Nyhan Syndrome, a genetic disorder that causes too much uric acid. More likely, the cause is the presence of excessive acids. Remember what I said? The task of the liver becomes heavier when excess acids are to be flushed out. In short, as long as there are excess acids, you will suffer from gout. As long as there are excess acids, there will be more tasks to occupy the liver, giving it less chance to flush out the unwanted uric acid.
