How Temperature Affects The Formation of Gouty Tophi?

Have you ever wondered why gouty tophy only found on certain parts of the body such as earlobe, fingers, elbow, knee, and almost only on the surface of the skin. Before I explain about this phenomena, I have to briefly discuss about the heat regulation of human body.

Every cell in the body does generate energy and some heat in the process of metabolism, but for a healthy human body, majority of the body heat is generated in organs that are constantly working such as heart, liver, and brain, thus the center of heat is in the torso and and middle of the head. But there must be some channels for the body to dissipate excess heat in order to maintain the body temperature at 37.5 degree Celsius.

Put a hand close to your mouth or nose and then slow breathe out, you can feel the warm air blowing on you hand, that is heat dispersion through breathing. If you dare, try to collect your pee in a paper cup and then dip your finger (gross disgusting away) into it and you will feel warmth, especially during the time when you are having fever. To feel the last channel of heat dissipation, which is the skin, put your right palm facing the your left palm and the slowly move them towards each other, you can feel the heat is building up in between your palms. As a matter of fact, most of the body heat is lost through the skin. 

That explains why the skin surface has lower temperature compare to other parts of the body. The temperature will drop lower when we are sleeping because the blood circulation will slow down. Gouty tophy is the result of crystallization of uric acid beneath the skin surface. If you remember your high school science, low temperature increases the rate of crystallization. That is why most of the gouty tophy are found on the skin surface, especially the areas that are not covered clothes such as earlobes and fingers.
